Help! Our office ran out of Arbitration Forms. How can we re-order?
For your convenience, the Order Form as well as the Arbitration Forms in various languages are available for you to download.
Forms must be printed in color to be considered legal and binding.

Arbitration Order Form

We just received notice of a potential claim. How do we report it?
In the event you directly or indirectly become involved in a professional liability claim, you should immediately report the details.
Failure to promptly report a claim may jeopardize coverage.

See our Reporting Procedures for general information, then:
  • Complete a Fairway Claim Form in its entirety
  • FAX the form to 800.859.7013 or Email the Claims Department
  • An Adjuster will be assigned to your case and will contact you within 48 hours of receipt. Feel free to call the office directly at 818.889.7399 if anything comes up in the meantime.
